Badge Request Form

Please fill out this form to request a new badge due to the following:

  • Employees with front-facing WashU Medicine IDs
  • Picture, name, or title is faded due to wear
  • The picture does not represent a current depiction of the holder
  • The badge is cracked or damaged
  • It’s been 4-5 years since your last issue

If any of the following are applicable to your badge request, please ask your department administrator to request one through ServiceNow:

  • Changes to your title and/or department
  • Legal changes to your name
